Every once in a while, maybe once every 2 or 3 weeks, I will get a period of rough idling after a warm start that usually lasts for about 5 to 7 seconds, or until I rev the engine a little. This never happens during a cold start, and seems to happen only during warm starts after the truck has sat for 1-2 hours. Any ideas???

Give it some gas for about 5-7 seconds on a warm start before driving off... Problem solved...

I would start with the basics if you have done it already :
1. Throttle Body cleaning
2. IAC clean out
3. Intake air temperature sensor
4. O2 sensors
5. Check plug gaps and wear state
That would be a starting point for me.
