Fuel pressure regulator???????

Stumbling rough idel rough driving and very rich. Blowing black smoke if I get on it. Getting 7.4 mpg...
I've replaced air filter
PVC valve
EGR valve
Cleaned the throttle body.
No luck....
Rented fuel pressure checker and with key just in on position I'm at 59 psi and it held that for 10 mins.
Turned truck on and the needle is all over both at idle and at 2500 rpm. This leads me to think fuel pressure regulator which is also the fuel filter I think.
What do you guys think check out the short clip.

That’s not an oil filled gauge so it’s normal to see it bouncing around like that.

With all of my steps taken what do you think it is?

Dirty injectors, spark plugs , map sensor, or vacuum leak.

My guess is MAP sensor has a problem..
